Maurice 5f4fa30416 docs: say where the outbound proxy switch actually lives ...

The variables only take effect when Node runs with NODE_USE_ENV_PROXY=1, and
that is set in the image. This page also covers Helm, Unraid and source
installs, where nothing sets it, so following it there changed nothing.

Also recommend NO_PROXY. Without it the proxy sees every request TREK makes to
itself, including the container health check.
