Maurice 2c3fb15263 style(server): format the files the health lanes added or moved
Prettier on every new or moved file that was not formatted, so lint:format
holds them; the files taken off the format baseline are the ones that moved
or were formatted on the way.

/**
* MfaPolicyGuard — the require_mfa policy, moved off the Express layer.
*
* The cases that matter here are the exemptions, because that is where the
* middleware had drifted: it decided on two literal path lists, and every public
* /api endpoint added after those lists were written answered a logged-in user
* without MFA with a 403 while answering a stranger fine. MFA-011 pins the
* correction.
*
* Plan 3b Task 1 (Task 0 review flag — the one guard test needing real
* rework): `MfaPolicyGuard` now takes `AppSettingsRepository`/`UsersRepository`/
* `WebauthnCredentialsRepository` instead of `DatabaseService`, so this file
* stubs the three REPOSITORY METHODS directly (`getValue`/`getMfaEnabled`/
* `hasAny`) rather than hand-rolling a `DatabaseService.get` stub that
* branched on SQL text substrings.
